Key features
The mark and its approved forms
The mark as it is to be used, registration details and where the approved forms, such as a style guide, are set out.
Exclusive, sole or non-exclusive
A blank for the kind of licence and what the licensor will and will not do itself in the territory.
Fees, sales reporting and checking figures
What is paid, how sales are counted, reports and payment, and the licensor's right to have records examined, with a threshold for shifting the cost.
Quality, samples and approvals
Standards the goods or services meet, samples before first use, approval of artwork, packaging and advertising, and what happens if the licensor does not reply.
Protection, sub-licensing and the end
Telling each other about infringement, no sub-licensing without consent, and selling off stock and removing the mark at the end.
About this document
A trademark licence agreement lets a licensee use a licensor's mark on and in connection with stated goods or services, in a stated territory and period, on payment. It sets out approved forms of the mark, quality standards and approval of material, reporting, record checks, protection of the mark and what the licensee does at the end.
This blank form is completed by hand. The mark stays the licensor's throughout, the licensee may not register it or challenge ownership, and the licensor controls quality. How much control is enough, and how licensing affects a mark's protection, are matters for a lawyer; the form does not decide them.

Questions
About the trademark licensing agreements
No. The form says the mark stays the licensor's and the licensee gets only the permission given.
