Assessment with examples of quality, volume, deadlines and independence, and of colleagues, customers, communication and feedback.

  • What went well and what next

The employee's account in their own words, what they want more and less of, where they want to get to and the support they ask for.

  • Development, objectives and pay

Training agreed, three objectives with success measures and dates, an overall assessment and whether it connects to a pay decision.

About this document

An employee evaluation records a review conversation: what was agreed at the start of the period, how each objective went, the work itself, working with others, what went well, what would make the biggest difference next, what got in the way, the employee's own account and wishes, training and development, objectives for the next period, an overall assessment and any link to pay.

This blank form is completed by the reviewer and employee together and signed by both, with a line for a second reviewer. It records a conversation so both have the same account; it is not a legal assessment. It says nothing in it is a promise of an increase or bonus. How evaluations may be used in employment decisions varies, so apply them consistently and fairly, and ask a lawyer where they feed into serious decisions.
