Four rows: what to do, what to stop, what to repair or pay for and written confirmation, with dates.
Cost, and what may follow

Not a notice to end
A statement that the tenancy continues, plus a way for the tenant to disagree or ask for time or help.
About this document
A lease warning notice tells a tenant that a term of the lease is not being kept, says what the landlord says has happened, what has already been said, what the tenant is asked to do and by when, any cost and what the landlord may do if it is not put right.
It is a blank form, a letter to complete by hand. The form states plainly that it is not a notice ending the tenancy. It is not a statutory notice to cure or quit either: states set their own requirements for notices before a landlord can take further steps, and a letter like this does not replace them.
