About this document
An invoice bills a customer for goods or services supplied. This form has a header for the invoice number, date and due date, a table of items, a block of totals, payment instructions and a way to raise a dispute.
It is a blank to complete by hand, with space for five lines of items and the usual totals. It does not decide tax or late-payment charges for you; those are for you to fill in according to your agreement and local rules.
