About this document
A waiver and release is signed by someone taking part in an activity. It records what the activity is, the risks involved, that the participant accepts them and that they give up claims against the organizer, other than for deliberate or reckless acts.
Whether and how far a waiver is upheld depends on the state, the activity and the facts, and some people cannot waive some claims. This blank form records what was explained and agreed. It includes a section for a parent or guardian where the participant is a minor.

Questions
About the liability waiver
No. It excludes claims for deliberate or reckless acts, and courts in some states are reluctant to uphold waivers. Get legal advice if you run higher-risk activities.
