About this document
A contract amendment records changes two parties have agreed to a contract they have already signed. It identifies the contract and any earlier amendments, numbers this amendment, says when the changes take effect and why they are made, lists the terms changed, added and removed, and deals separately with price or payment, dates or duration, what is supplied and the parties' details.
This blank form is completed by hand and signed by both parties and, where one is needed, a guarantor or other consenting party. It confirms that the rest of the contract stays in force, that each signer is authorised, that nothing is given up unless it says so, and that the amendment is kept with the contract. Whether the contract itself requires a particular method or anyone else's agreement to change it depends on its terms, so read it first.
