About this document

A release of lien is given by a claimant, such as a contractor or supplier, who has claimed a lien on a property. It identifies the lien by its recording details, amount and origin, the property, what has been paid, and releases the lien in full or to a stated extent.

This blank form is signed by the claimant and the owner, with an optional witness line and a notary page. It states whether anything remains claimed, that the claim has not been assigned, that those the claimant engaged have been paid so far as it knows, and that it releases that lien only. Whether a release works, and how and where it must be recorded, depends on the state and the kind of lien.
