A commercial lease application is a form a business fills in to ask to lease premises. You prepare it with the property and the proposed terms, and the tenant completes the details of the business, its owners, its bank and its references.

This builder asks for the landlord or property manager, whether to ask for driver's license and passport details, the property and any suite or bay, the leased area, the term and possession date, the base rent and which operating costs the tenant bears, advance rent and a deposit, leasehold improvements and any additional terms. The tenant's sections are blank lines to complete: business details, up to three owners, banking, credit and other references, a permission to contact references and a credit check authorization.

The form does not say what the law allows a landlord to ask or check. That depends on your state, and a lawyer can tell you.
