About this document
A childcare agreement records the arrangement between a parent and the person looking after their children: the children, where the care is given, days and hours, pay, expenses, duties and boundaries, health and emergencies, time off and how either side may end it.
This blank form includes a status line for whether the provider is an employee or works for themselves, and says each party is responsible for their own obligations that follow from that. How a carer is classified, and what employment, tax and registration rules apply, varies by state, so check before you rely on it.

Questions
About the childcare agreements
The form has a blank for what the parties intend. How a carer is treated depends on the facts and the state, and tax and employment obligations can follow. Ask an adviser.
