About this document
A remote work policy sets out who may work away from the premises, how an arrangement is agreed and recorded, when it can be changed or withdrawn, where the work may be done, hours and keeping in touch, how work is reviewed, equipment, costs, working environment and safety, information security, coming in and staying part of the team.
This blank form is completed by the employer. It says it does not change anyone's employment agreement. It does not say who is entitled to work remotely or who bears which cost: those are blanks. Working from another state or country can change an employer's tax, insurance and legal obligations, and the form says it needs separate consideration, so take advice before agreeing it.

No. Eligibility is a blank. In some places there are rights to request flexible working or to reasonable adjustments, so check what applies.
